The Lung & Heart Transplant Foundation Assistance Fund is intended to be a charitable provider of limited financial assistance to lung and/or heart transplant patients. Assistance provided will be limited to medication co-pays, uncovered prescription medications, transportation to or from the patient’s home and/or the transplant center as well as lodging expenses directly related to transplant care.

Eligibility:  (to apply for assistance all applicants must meet the following criteria)

• Short form is to be used only for one-time requests of $500 or less.

• Must be either post lung and/or heart transplant or on a waiting list for a lung and/or heart transplant.

• Must have a medical or other related need for financial assistance which is of an emergency or non-

  insured nature.

• Must complete all sections of the application.

• The application for assistance must be verified by one of the following members of your transplant team:

  Social Worker, Transplant Nurse Coordinator, or Financial Coordinator.

• All documentation is to be submitted as requested and all appropriate pages must be signed.

• Applications are required to be submitted by the lung or heart transplant program.
