Short form is to be used only for one-time requests of $500 or less. Must be either post lung and/or heart transplant or on a waiting list for a lung and/or heart transplant. Must have a medical or other related need for financial assistance which is of an emergency or non- insured nature. Must complete all sections of the application. The application for assistance must be verified by one of the following members of your transplant team. (Social Worker, Transplant Nurse Coordinator, or Financial Coordinator) All documentation is to be submitted as requested and all appropriate pages must be signed. Applications are required to be submitted by the lung or heart transplant program.
